Position Overview:
We are seeking a highly organized and detail-oriented RN Care Coordinator to join our Johns Creek, GA practice. This role is vital to ensuring that patient care plans and physician orders are executed efficiently while maintaining high clinical standards and regulatory compliance.
The RN Care Coordinator will work closely with patients, families, providers, and external healthcare organizations to oversee mobile wound care operations, patient scheduling, treatment coordination, and quality assurance initiatives. This position requires strong communication, problem-solving skills, and clinical expertise to optimize patient outcomes while maintaining efficient workflow processes.
Key Responsibilities:
Care Coordination, Quality Assurance, and Scheduling:
- Perform intake on all new referrals and schedule consultations for patients with chronic wounds and pressure ulcers.
- Develop and maintain patient schedules while optimizing travel routes for mobile wound care teams.
- Review and update patient records to ensure accurate documentation of wound assessments, treatment plans, and physician orders.
- Monitor patient progress and investigate adverse wound-related events to improve clinical outcomes.
- Collaborate with the Medical Director and multidisciplinary team to develop personalized discharge plans.
- Coordinate medical records, physician orders, and required authorizations to facilitate seamless patient care.
- Work closely with the Authorization Specialist to confirm insurance coverage benefits and secure prior authorizations.
- Ensure compliance with Restore First Health’s policies, standards of care, and regulatory requirements.
- Oversee wound care documentation, referral criteria, supply inventory, and tissue tracking.
- Maintain records for wound care products, amniotic allografts, and tissue IDs in compliance with regulatory standards.
- Work collaboratively with wound care providers and external healthcare entities to ensure comprehensive, evidence-based wound management.
- Partner with home health agencies, hospitals, and long-term care facilities to coordinate continuity of care.
- Educate patients and families about Advanced Wound Therapy and its role in comprehensive wound care management.
- Serve as a primary clinical consultant for the Medical Director and wound care providers.
- Train facility staff and providers on wound assessment best practices and protocol implementation.
- Provide in-service education on wound care products, treatment strategies, and wound healing processes.
- Engage in outreach efforts to build strong relationships with referring physicians, home health agencies, and other healthcare partners.
- Active RN License in state of Practice (in good standing).
- Nursing degree from an accredited college or university.
- Minimum of 2 years of nursing experience (wound care, case management, or care coordination experience preferred).
- Strong organizational, problem-solving, and time-management skills.
- Excellent verbal and written communication skills.
- Ability to analyze patient data, identify care needs, and implement clinical best practices.
- Experience with electronic medical records (EMR) systems. (We use Athena)
- Knowledge of medical terminology, care coordination workflows, and insurance authorization processes.
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
- Monday – Friday, daytime hours.
- Full-time, mobile wound care setting.
