Job Requirements
Driver's License / State ID
Prime Drug Screening (minimum 10-panel test) - Within 60 days of start.
Prime Color-blind test - Must be completed annually.
License / Certificate Verification - Primary Source Verification of professional license, and / or certification, as required for the position. Ran within 30 days of start. Updated upon expiration.
COVID-19 Vaccine - 2 COVID-19 vaccines and booster required or declination completed
MMR - Proof of 2 Vaccines or Positive Titer showing immunity
Fit Test - Annual Respirator fit test.
Hepatitis B - Declinations not accepted at Shasta Regional Medical Center.
Influenza Vaccine - Flu Vaccine or declination required October1-May 31. Declinations not accepted at Shasta Regional Medical Center.
TB Test - Required.
Varicella - Vaccination / titer showing immunity
Tdap - Declinations not accepted at Shasta Regional Medical Center.
Annual TB Questionnaire - Required.
Physical / Health Statement - Annually, Except Saint Marys Regional Medical Center- Nevada must be completed within 60 days of start. Must state clear to work.
Additional Details
National Criminal Background Check within one year of start date.
