REFERRAL MANAGER
REPORTS TO: Admissions and Outreach Director FLSA STATUS: Exempt
SCHEDULE: Monday-Friday 8 a.m. – 5 p.m
POSITION SUMMARY:
The Referral Manager is a very important role within the Revive Ministries Inc. (Revive) organization. Referral managers represent the treatment programs offered by Revive and often become the face of Revive to many organizations.

The Referral Manager thrives when providing those struggling with substance use disorder an efficient placement in the appropriate treatment program.
GENERAL RESPONSIBILITIES:
1. Identify and build strong relationships with all applicable entities in the assigned Region
2. Educate entities on the history, programs and outcomes provided by Revive
3. Field calls from potential clients who are exploring treatment options
4. Manage travel schedule to efficiently visit entities that are potential referral sources and/or influential in the SUD field.
5. Maintain database on all activity, patient referrals, clients admitted in Best Notes
6. Communicate all new patient/client referrals and any potential barriers to Intake Team via MS Teams
KEY PERFORMANCE INDICATORS
1. Attend all meetings
2. Arrange and facilitate 4-5 face to face meetings per week with potential referral sources or entities that influence the SUD field (See examples below)
3. Have 5 meaningful phone contacts per day
4. Maintain daily travel schedule
5. Set time blocks in Outlook for making calls and administrative duties
6. Facilitate best treatment option for those suffering from substance use disorders
7. Update Best Notes and/or Shared File daily
8. Proactively report barriers, concerns, or schedule changes via MS Teams to Intake Team and ensure that the applicable Director or the COO is aware of trending barriers.
9. Turn in all receipts within 24 hours
BACKGROUND/ EDUCATION:
1. Preferred associate or bachelor’s degree in related field and 2 years of experience networking with agencies that provide or partner with substance use disorder programs.
QUALIFICATIONS REQUIREMENTS:
Must have knowledge of medical privacy and confidentiality (Health Insurance Portability and Accountability Act [HIPPA]); computer applications/software to include Microsoft Office programs/MS Outlook (e-mail), and internet familiarity.
Experience working with individuals and families who have various mental, behavioral, and substance abuse conditions.
SKILLS REQUIRED:
1. Must have organizational and communication skills, both written and verbal.
2. Communicate effectively in oral and written format.
3. Maintain effective interpersonal relationships.
4. Must have ability to work independently and work in a stressful and busy environment.
5. Work collaboratively with the prospective client and necessary internal departments to ensure an efficient admission process.
WORKING CONDITIONS:
Physical Demands:

Must be able to sit, stand, bend, reach overhead and lift up-to 10 pounds; travel outside of the office and/or attend meetings both during and after normal work hours, both at and away from the work site. Exerting up to 10 pounds of force occasionally or a negligible amount of force frequently to lift, carry, push, pull, or otherwise move objects, including the human body. May involve walking or standing for brief periods of time. Requires about 6 hours of sitting and no more than 2 hours of standing or walking in an 8-hour workday. No significant stooping is required.
Environmental Conditions:
The employee will work indoors in a residential setting with central heating and air conditioning. The house itself is open and ventilated well with central heating and air conditioning. The person may work outside in heat/cold, wet/humid, and dry/arid conditions during times of community activities for clients.
Mental Demands:
There are several daily deadlines associated with this position, which may cause significant levels of stress for some individuals. The position must deal with the clients, staff, and insurance representatives daily on a variety of issues.
WORK EXPECTATIONS:
1. Minimum of 40 hours per week
2. Must be able to work well with other staff and clients.
