Job Overview
We are seeking a dedicated and compassionate Care Manager to join our healthcare team. Responsibilities
- Coordinate and manage patient care plans, ensuring that all aspects of care are addressed.
- Conduct assessments to determine patient needs and develop individualized care strategies.
- Maintain accurate and up-to-date medical records in compliance with HIPAA regulations.
- Collaborate with healthcare providers, including hospitals and acute care facilities, to facilitate effective patient transitions.
- Utilize ICD-10 coding for accurate documentation and billing processes.
- Provide education and support to patients regarding their treatment plans and available resources.
- Monitor patient progress and adjust care plans as necessary to achieve optimal outcomes.
- Advocate for patients' needs within the healthcare system while maintaining a focus on quality service.
Requirements
- Proven experience in case management or a related field within healthcare settings.
- Strong knowledge of HIPAA regulations and patient confidentiality practices.
- Familiarity with medical records management and coding systems, including ICD-10.
- Experience in behavioral health or acute care environments is highly desirable.
- Excellent communication skills, both verbal and written, with a focus on patient service.
- Ability to work collaboratively within a multidisciplinary team while maintaining professionalism.
- Strong organizational skills with attention to detail in managing multiple cases simultaneously.
- A degree in nursing, social work, or a related field is preferred but not mandatory.
