What are the responsibilities and job description for the Nurse Practitioner: CRNP- Hospice position at Revolutionary Home Health?
Revolutionary Home Health & Hospice is looking for Certified Registered Nurse Practitioner to assist with patient care coordination duties of its growing hospice census. The CRNP works in conjunction with the Clinical Coordinator and other members of the hospice interdisciplinary team to assist in the processes of patient evaluation, care planning, and intervention. This position requires travel as patient care is provided to the patients in their homes.
Responsibilities of the CRNP Include:
1) Conduct initial and ongoing assessments to determine patient eligibility criteria
2) Assist in the care planning process of patients admitted to hospice services
3) Participate in bi-weekly interdisciplinary team meetings to discuss patient status, areas of focus, and requirements for updated interventions and care planning
4) Work in conjunction with the Hospice Medical Director to coordinate medication orders for patients as applicable to hospice diagnosis'
5) Additional patient-centered duties as directed by Home Health & Hospice Clinical Coordinator

Position Requirements Include:
1) Licensed as a Certified Registered Nurse Practitioner in the state of Pennsylvania
2) Prescriptive Authority and DEA Number
3) Valid drivers license in the state of Pennsylvania and reliable transportation (required to make patient visits as needed)
3) Prior experience in hospice is desired, but willing to train committed candidates
