Monte Vista is a site based Rapid Rehousing Program for families who have experienced homelessness. The program provides wraparound support to assist families with finding stability in housing and ensuring long term success.
Position Summary
The Housing Case Manager will assist clients with the development of social and community support systems to promote stability and enable clients to function at the highest level possible. The Housing Case Manager will work with clients in the rapid rehousing program and assist clients with transitioning into permanent housing. The Housing Case Manager will assist clients with obtaining documents needed for housing, locating appropriate housing, increasing income, and remaining successfully housed. The Lead Housing Case Manager is a senior role that will be tasked with the most challenging cases and providing mentorship to Housing Case Managers, the ideal candidate has a desire to move into program leadership roles.
Essential Duties and Responsibilities
The essential functions include, but are not limited to the following:
- Coordination of services for client
- Ability to provide independent life skills training, group education, and family activity coordination.
- Assist clients with gathering essential documents for housing
- Locate housing opportunities in the community
- Maintain appropriate client files and complete documentation within the required timeframe
Minimum Qualifications (Knowledge, Skills, and Abilities)
- 2-3 year experience in housing related social services
- Cleaning driving record
Salary : $27 - $28
