IN-HOUSE PHARMACIST
JOB SUMMARY:
An in-hospital pharmacist plays a critical role in ensuring safe and effective medication use within a healthcare facility. The pharmacist is responsible for preparing, dispensing, and monitoring medications to ensure optimal patient outcomes. They collaborate with healthcare professionals to develop and implement individualized treatment plans, educate patients and staff about drug therapies, and maintain compliance with regulatory standards. The role includes evaluating medication orders, preventing drug interactions, managing inventory, and participating in clinical rounds to provide pharmacological insights.
SUPERVISION RECEIVED: Reports directly to Chief Nursing Officer
SUPERVISION EXERCISED: Pharmacy Nurse
ESSENTIAL FUNCTIONS:
- Medication management:
- Manage ordering and purchasing medications
- Monitor drug expiration dates
- Maintaining hospital inventory
- Ordering specialty medication
- Pharmacist may be on-call or located elsewhere to answer questions or provide medications
- Obtain an accurate and complete patient medication history, aka “Med Rec”, including prescription drugs, over-the-counter medications, others (alcohol, cigarettes, illicit drugs) by:
- Interview patients and family members
- Call the patient’s retail pharmacy
- Utilize third-party applications for refill information
- 340B medication program collaboration
- Report drug shortages
- Comply with drug recalls
- Optimize the outcomes of patients with critical illness by providing evidence-based, patient-centered drug therapies, and monitors for drug-related problems and medication cost containment
- Provide pharmacokinetic consultations as required
- Detect, monitor, document, and report adverse drug reactions and medication errors
- Monitor drug therapy to evaluate the appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions.
- Communicate ongoing patient information and efficacy and safety of treatment regimen(s) to all care providers
- Provide in-service education to the care team, perform patient and caregiver education as appropriate, and counsel patients regarding medication therapy and compliance as needed.
- Lead and participate in interdisciplinary hospital committees, protocol development, and education of pharmacists, nursing and physicians.
- Leads antimicrobial stewardship efforts within the hospital.
- Utilizes departmental and hospital processes and systems to insure correct distributive and clinical pharmaceutical care activities.
POSITION QUALIFICATIONS
- Minimum Education
- Graduate from an accredited school of pharmacy
- Minimum Experience
- Two years of experience as registered pharmacist in a hospital setting desired
- Current license as Registered Pharmacist with State of Kansas required.
