Job Summary
The Lead MNCHOICES Certified Assessor will assist with mentoring, on-boarding new staff, and monitoring / approving all CFSS (Consumer First Services and Supports) plans. This position will be able to conduct assessments if needed, provides for on-going training and updates to assessors and case managers for all information pertaining to MNCHOICES. They will attend state and county trainings on MNCHOICES, become the liaison to DHS for MNCHOICES, and attend inter-disciplinary meeting with Rice County Public Health regarding long-term care services.
Examples of Duties
- Completes MnCHOICES assessments for individuals to identify appropriate options for care.
- Provides case management services to individuals under the age of 65 residing in Rice County receiving CFSS services, and not on a Minnesota Waiver Program.
- Attends training sessions and staff / community meetings.
- Assure training of staff in all areas to assure compliance.
- Performs other duties as assigned.
Qualifications
Minimum Qualifications : This position requires a Bachelor's degree in Social Work or related human services field, and two years of supervised experience working with individuals with physical disabilities, mental illness, brain injuries or developmental disabilities. A valid Driver's license is required upon hire. The Lead Social Worker must be a MNCHOICES Certified Assessor through the State of Minnesota.
Preferred Qualifications : A Master's degree in Social Work, and three years of experience as a County Social Worker is preferred.
Supplemental Information
HIRING RANGE / UNION : $37.14 - $38.82 per hour / SS unit - Grade 13
HOURS : Business hours are 8 : 00am - 4 : 30pm, Monday - Friday. Full-time hours equal 40 hours per week.
