Job Summary
**FIRST REVIEW OF APPLICATIONS JANUARY 27TH
The Developmental Disabilities Social Worker coordinates assessments of clients' functional skills and needs and assesses the supports and services available to meet needs and preferences. Develops, revises, and implements individual service plans and ensures coordinated approaches to services among all providers. Provides guardianship services to public ward. Completes MnChoice Assessments annually for each client as needed.
Examples of Duties
- Provides case management services to assist clients with developmental disabilities in meeting their highest possible level of independence, following the established rules as outlined in Rule 185.
- Manages funds for clients who are unable to do so themselves. Manages waiver and service agreements, authorizes purchases of services, monitors expenditures, participates in contract negotiations, and directs representative payee accounts.
- Attends training sessions and staff/community meetings
- Transports clients to appointments and travels to client meetings; attends medical and psychiatric appointments with clients, within and outside of County.
- Provides back-up intake support for the County and responds to calls on the County crisis line; follows up on child protection and/or vulnerable adult reports, investigations, etc. to ensure that client risks are minimized.
- Performs other duties as assigned.
Qualifications
The job requires a bachelor's degree in Social Work or a related human services field and two years of supervised experience working with people with developmental disabilities. A valid Driver's license is required upon hire. MnChoices assessment certification required by the State of Minnesota.
PREFERRED QUALIFICATIONS:
Master's degree in Social Work, and three years experience as a county Developmental Disabilities Social Worker preferred.
Supplemental Information
HIRING RANGE / UNION: $34.79 - $36.37 per hour / SS Unit – Grade 12
HOURS: Full-time hours equal 40 hours per week. Business hours are 8:00 am - 4:30 pm, Monday – Friday.
