This position requires a high school diploma or GED and one year of post-secondary education. Applicant must have a community health worker certificate from the State of Minnesota (one-year on-line program) at hire, or within one year of hire. One year to three years of experience in health education or a related field and experience working with culturally diverse families. A valid driver’s license is required upon hire. Bilingual in English and Somali.
PREFERRED QUALIFICATIONS:
Associate or bachelor's in health-related field. Prior work experience in health care field.UNION: GU Union – Grade 7
HOURS: 30 - 40 hours per week (.6 FTE - 1.0 FTE). Flexible work hours typically between 8:00a.m - 4:30p.m., AND 3-5 days per week. Occasional weekend or evening hours.
The Community Health Worker (CHW) independently provides direct face-to-face services to teach health and safety information or parenting skills to families with young children in home, office, or community settings. The Community Health Worker will provide ongoing family home visiting to assigned families, provide family support, and actively assist in making referrals to community resources. The Community Health Worker in this position will also provide community outreach, gather community input, and support community health equity work. CHW’s also may conduct community outreach events, and public presentations. The CHW works under the direction of a public health nurse or unit supervisor. |
- Independently provides ongoing family home visiting to pregnant women and families with young children.
- Completes required paperwork including documentation of client visits.
- Connects families to community resources and interacts with staff from a variety of agencies.
- Provides community engagement, outreach, networking and health promotion, & prevention education to various diverse communities, and hard to reach populations.
- Conducts community outreach events.
- Assists with agency interpreting or translation needs.
- Performs other duties as assigned.
