The Adult Mental Health Social Worker assists adults with Serious and Persistent Mental Illness (SPMI) diagnosis in achieving and maintaining mental health stability and maximum independent living within the community. The Social Worker helps adults with SPMI gain access to needed medical, social, educational, vocational, and other necessary services as they relate to his/her individual mental health needs.
- Meets with clients during regular home visits to develop relationships, assist them, and support them in reaching their goals.
- Completes and maintains required documentation for casework. Documentation includes but is not limited to SSIS, service arrangements, dictation, and referral forms.
- Communicates with clients, service providers, and other involved parties. Schedules appointments and makes referrals to outside organizations as needed.
- Completes functional assessments and develops and implements Individual Community Support Plans for each individual served.
- Coordinates client transportation to appointments as needed and attends client meetings in and out of County.
- Attends training sessions and staff/community meetings.
- Performs crisis intervention as needed.
- Performs other duties as assigned.
MINIMUM QUALIFICATIONS:
This position requires a Bachelor’s degree in Social Work or related human services field, and two years supervised experience working with adults in the mental health field. A valid Driver’s license is required upon hire.
PREFERRED QUALIFICATIONS:
A Master’s degree in Social Work, and three years of experience working in County social services preferred.
HIRING RANGE / UNION: $34.79 - $36.37 per hour / SS Unit – Grade 12
HOURS: Normal working hours are 8:00 am - 4:30 pm, Monday – Friday. Full-time hours equal 40 hours per week.
