The CAC/CADI/TBI Social Worker coordinates the assessment of clients' functional skills and needs and assesses the supports and services available to meet client needs and preferences. Develops, revises, and implements individual service plans and ensures coordinated approaches to services among all providers.
This position requires a Bachelor's degree in Social Work or related human services field and two years' supervised experience working with people with physical, mental and/or developmental disabilities. In addition, one year experience working with Home and Community Based Waivered services. Valid Driver's License required.
PREFERRED QUALIFICATIONS:
Master's degree in Social Work and three years of experience as a county Social Worker in the areas of Developmental Disabilities or CAC/CADI/TBI services. MNChoices certified Assessor through the State of Minnesota. Bi-lingual in Somali or Spanish.HIRING RANGE / UNION: $34.79 - $36.37 Per hour / SS Unit - Grade 12
HOURS: Full-time hours equal 40 hours per week. Business hours are 8:00 am - 4:30 pm Monday - Friday.

- Assesses and evaluate client needs through the MNChoices Assessment.
- Provide case management services in an ethical manner following the established CM rules.
- Administer services according to client need and funding availability.
- Review, analyze and follow-up on client progress reports, medical and psychiatric reports, therapy reports, behavioral reports, and auxiliary service reports.
- Authorize the purchase of services to meet identified client needs.
- Educate clients, families, and providers regarding service options and available supports and funding mechanisms.
- Coordinate, develop, and perform service planning and monitoring.
- Assess and evaluate client needs.
