**POSITION IS OPEN UNTIL FILLED
The Social Worker of Supported Decision Maker (SDM) is responsible for assisting individuals who do not meet the statutory criteria of "incapacitated" for guardianship or conservatorship. This position will work to assist those who voluntarily agree to supported decision making. This position will also be required to educate our community on Support Decision Making (SDM) by meeting with providers and/or families.
HOURS: Full-time (40 hours per week). Typical work hours between approximately 8:00 a.m. - 4:30 p.m. Monday - Friday. Flexible scheduling. May occasionally work limited hours on weekend, holiday, or evening.

- Assists individuals needing services and supports, access, information, and referrals.
- Attend various meetings; gathers data and completes necessary documentation and reporting as required per the State grant within the set timelines.
- Attends required training sessions and meetings as directed.
- Performs other duties as required.
MINIMUM QUALIFICATIONS:
Bachelor's degree in Social Work or a related field and two years of experience working with vulnerable individuals. Valid driver's license required.
PREFERRED QUALIFICATIONS: Master's degree in Social Work, and previous experience as a county social worker is preferred.
