Introduction:
We are seeking a dedicated and compassionate Chronic Care Management Medical Assistant (MA) to join our healthcare team. The ideal candidate will assist in providing comprehensive care for patients with chronic conditions, working closely with the healthcare team to manage and improve patient outcomes. This role will involve both administrative and clinical duties to support chronic care management programs.
Key Responsibilities:
- Patient Support: Assist in coordinating care for patients with chronic conditions (such as diabetes, hypertension, COPD, heart disease) by performing routine checks, health assessments, and providing education on disease management.
- Care Coordination: Schedule and follow up on patient appointments, laboratory tests, and other diagnostic procedures. Ensure patients adhere to treatment plans and help them manage their conditions through regular check-ins and follow-ups.
- Health Monitoring: Record and monitor patient vital signs, symptoms, and progress. Track health data and assist providers with the management of care plans.
- Documentation: Maintain accurate and up-to-date patient records, documenting all interactions and care plans in the electronic health record (EHR) system. Ensure compliance with healthcare regulations.
- Patient Education: Provide education to patients and their families on managing chronic conditions, medication adherence, and healthy lifestyle choices. Answer patient questions and provide resources as needed.
- Collaboration: Work closely with physicians, nurses, case managers, and other healthcare professionals to ensure comprehensive care for patients. Communicate effectively within the healthcare team to promote patient outcomes.
- Administrative Tasks: Perform office-based administrative duties such as handling phone calls, confirming appointments, insurance verification, and patient intake.
