Position Overview
We are seeking 2–3 Licensed Practical Nurses (LPNs) with home health experience to join our team. In this role, you will travel to patient homes to provide wound care, administer medications and injections, and update patient records. You will enjoy a high level of autonomy while working closely with our RN case managers and physicians. Strong critical thinking, assessment skills, and self-sufficiency are keys to success.
Key Responsibilities
- Provide skilled nursing services, emphasizing wound care, medication administration, and injections
- Perform patient assessments and promptly communicate changes to RN case managers or physicians
- Accurately document each visit and maintain up-to-date medical records
- Collaborate with the interdisciplinary team to deliver comprehensive care
Qualifications
- Active LPN license in Colorado
- Must have some type of prior home health experience
- Strong critical thinking, assessment, and problem-solving skills
- Ability to work independently, travel between patient locations, and manage time effectively
