SUMMARY
The Care Partner III is responsible for providing front-line leadership in providing resident care and administration of medications to residents as ordered by the attending physician. The candidate must be a licensed QMAP, as well as a CNA, or obtain certification within 1 year of hire. Responsible for working in the role of Care Partner II/QMAP and/or Care Partner I, as needed. Care Partner III will assist with the training of Care Team members and be looked upon to set an example for others in the Department. Scheduling flexibility is necessary in this position. In addition, this position will be assigned various departmental tasks as determined by the Assisted Living Director and/or Memory Support Director. Reports to: Assisted Living Director or Memory Support Director, as assigned. Hours are as follows: Saturday & Sunday, 3pm - 2am, Monday & Tuesday, 2pm - 1am.

1. Serve as a supervisor for the assigned shift. Ensure that resident care and medications are delivered timely, thoroughly and in compliance with all doctor’s orders, community/company policy and state regulations. This supervisory status is in addition to working on the floor as a Care Partner II/QMAP and/or Care Partner I.
2. Serve as a role model for all care team members in job performance, attendance/tardiness, willingness to help others, attitude and professionalism.
3. Effectively manage staff call-offs for all assisted living or memory care (as assigned) staff. Carry and answer the Scheduling Phone while working. Work to fill open shifts, as needed, using On-Shift (or other scheduling software) and directly contacting team members. Primary responsibility for filling open shifts due to call'offs is the responsibility of the Care Partner III.
4. Assists residents with their daily medications, treatments and resident care as assigned and within the scope of practice, policy, procedures, and regulations
5. Provide input to Assisted Living Director, Memory Support Director or Team Operations Director regarding the performance of all care team staff members. May be asked to participate in disciplinary/counseling sessions with care team members.
6. Keep medicine carts and areas orderly, clean and appropriately stocked and locked. Maintain upkeep and cleanliness of medicine carts and medication rooms.
7. Assist care partners in the performance of their job and assume care partner activities when needed or requested.
8. Prepare medication for distribution to residents as per the Company’s Medication Policies.
9. Assist, as directed, with training of inexperienced staff members in the department. Help inexperienced staff members assimilate into the department by serving as a resource for them.
10. Perform duties and display personal character in such a manner that serves as a professional role model for others in the Department.
11. Involvement and/or leadership of both departmental and/or community-wide committees and projects.
12. Provide supervision, management and leadership to Care Partner II/QMAP and Care Partner I in a manner that follows all Living the Brand principles and encourages professional development.
13. Communicate with physicians and family members as appropriate.
14. Observe and communicate with Assisted Living Director and/or Memory Support Director any changes in resident physical or mental status.
15. Carry a radio or communication device as directed by supervisor to ensure prompt response to resident issues.
16. Demonstrate knowledge of infection control, i.e., adequate handwashing, blood borne pathogens procedures, universal precautions, etc.
17. Maintain all required licenses/certifications as required.
18. Report occupational exposure to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the community’s policies and procedures governing accidents and incidents.
19. Provide support to community’s marketing efforts through positive/friendly interaction during all community tours and telephone contact with the public, and as directed by a supervisor.
20. Maintain a safe and secure environment for all staff, residents and guests, following established safety standards.
21. Encourage teamwork through cooperative interactions with co-workers and other departments.
22. Support a positive and professional image through actions and dress.
