Position Summary:
The Hospital/Clinical Liaison is responsible for managing referrals and business development for their accounts and area. Maintaining excellent customer service is our goal.
Education & Responsibilities:
- MUST HAVE experience in skilled nursing facility admission/ marketing and referral assessments.
- Have the ability to work in a fast paced environment, have excellent organizational and communication skills.
- Have clinical knowledge and be able to read referrals, assess patient needs and assist in making clinical acceptance decisions for our communities.
- Work to grow market share by increasing conversions from hospitals to our communities.
- Provide onsite support at hospitals and assist potential clients with making their community of choice and assist case management with finding the appropriate skilled nursing community for their referrals.
- Engage facilities with the communities they serve by providing outreach programs to local senior organizations and senior living communities.
- Work with communities to build and work business development plans to achieve census goals as well as increase referral sources and overall market share.
Experience:
- Previous experience in long-term care required.
- Licensed Nurse or Therapist a plus but not required.
- Excellent skills in communication, management, organization, and problem-solving
- Coordinating with hospital, family, and physicians
- Position is expected to travel
