About the Role:

The Resident Care Coordinator plays a pivotal role in ensuring the highest quality of care for residents at The Ridge at Lansing. This position is responsible for developing, implementing, and overseeing individualized care plans that meet the unique needs of each resident. The coordinator will work closely with healthcare professionals, families, and residents to facilitate communication and ensure that care is delivered effectively and compassionately. Additionally, the role involves monitoring the progress of residents and making necessary adjustments to care plans as needed. Ultimately, the Resident Care Coordinator is dedicated to enhancing the overall well-being and quality of life for all residents in the community.

Minimum Qualifications:

  • Bachelor's degree in nursing, social work, or a related field.
  • Valid nursing license or certification in the relevant field.
  • Experience in a healthcare or residential care setting.

Preferred Qualifications:

  • Previous experience in a care coordination role.
  • Certification in gerontology or a related specialty.
  • Familiarity with electronic health record (EHR) systems.

Responsibilities:

  • Develop and maintain individualized care plans for residents based on their specific needs and preferences.
  • Coordinate with healthcare providers, families, and staff to ensure seamless communication and delivery of care.
  • Monitor residents' health and well-being, making adjustments to care plans as necessary.
  • Conduct regular assessments and evaluations of residents to track progress and identify any changes in condition.
  • Provide training and support to staff on best practices in resident care and ensure compliance with regulatory standards.

The required skills for this position include strong communication and interpersonal abilities, which are essential for building relationships with residents, families, and healthcare providers. Organizational skills are crucial for managing multiple care plans and ensuring that all aspects of resident care are addressed in a timely manner. Problem-solving skills will be utilized daily to address any challenges that arise in the care process and to adapt plans as needed. Additionally, empathy and compassion are vital in providing support to residents and their families during difficult times. Preferred skills, such as familiarity with EHR systems, will enhance the efficiency of documentation and care coordination efforts.
