Care Coordinator for Mobile Health Clinic

RIDGEVIEW, a not-for-profit community mental health center, is pleased to announce an opportunity to be a part of our Mobile Health Clinic to the right bachelor's level candidate.  This innovative team-based program, which provides integrated primary care and behavioral health services to rural locations, is seeking a Case Manager/ Care Coordinator to serve as a key component of providing those services. This position will work weekdays during normal business hours with some flexibility.

A highly specialized and customized vehicle (aka Mobile Health Unit) serves as the base of operations for the Case Manager as well as the other members of the team (i.e. Nurse Practitioner, Therapist, etc). The MHC multidisciplinary team travels to designated locations within Ridgeview’s service area in order to provide increased access to basic health care, mental health and substance use treatment in historically underserved communities. The program utilizes a team-based model of care which includes mental health counseling, primary health care, case management/care coordination, supportive employment services and access to a peer recovery specialist.  We are excited about this opportunity to bring hope, healing and recovery to Fentress, Campbell, Scott, Morgan, and Union counties in the form of a Mobile Health Clinic.

REQUIREMENTS

• Bachelor's degree in the social sciences or a mental health field (e.g. counseling, psychology, social work, vocational rehabilitation) or Bachelor's degree transcript including 15 credit hours of related courses
• Prefer experience working with individuals with severe and persistent mental illness, but new grads are encouraged to apply
• Must maintain a valid TN driver's license and a safe driving record
• The position requires a self-starter with excellent time management skills

JOB DUTIES

Care coordination is a process for directing and coordinating services to consumers of mental health services.  The Care Coordinator works directly with youth and/or adult clients, their families, and community resources both in and out of Ridgeview to secure necessary services to meet client needs.
