Purpose of Job
This position is responsible for coordinating patient care assignments, answering the phone and other administrative duties to support the Home Health Department.
Job Functions
- Coordinates and manages patient care assignments for all Home Health professionals and paraprofessionals.
- Communicates and coordinates referrals with Intake Staff.
- Serves as a liaison between clients and staff as well as staff, physicians, pharmacy, lab, supplies and/or messages as appropriate.
- Provides customer service to internal and external customers by answering inquiries or referring to the appropriate staff personnel.
- Participates in on-call staffing rotation according to department needs.
- Assists in record keeping.
- Performs other duties as assigned.
Minimum Education/Work Experience
- High school graduate or equivalent
Knowledge/Skills/Abilities
- Detail oriented and organized
- Ability to meet and maintain the necessary background checks as aligned with position functions
- Ability to work and multi-task in a fast paced environment
- Ability to work autonomously and as a team
- Excellent written and verbal communication skills
- Ability to communicate in the English language for effective written and verbal correspondence in order to complete job functions as mentioned above
Preferred Qualifications
- Scheduling/staffing experience in a health care system
- Microsoft Office experience
