About the Role:
We are seeking a highly motivated and skilled Community Health Worker to join our team in the Health Care Services industry. As a Community Health Worker, you will be responsible for providing support and guidance to individuals and families in the community to improve their overall health and well-being. You will work closely with healthcare professionals to identify and address health disparities and promote healthy behaviors. Your role will be critical in ensuring that our community members receive the care and resources they need to lead healthy and fulfilling lives.
Minimum Qualifications:
- High school diploma or equivalent.
- Minimum of 1-3 years of experience in public health, mental health, case management, or healthcare support preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
Preferred Qualifications:
- Bachelor's degree in public health, social work, or a related field.
- Certification as a Community Health Worker.
- Experience working with underserved populations.
- Knowledge of local community resources and services.
- Experience with data collection and analysis.
Responsibilities:
- Conduct outreach and engage with community members to assess their health needs and provide education on healthy behaviors and disease prevention.
- Collaborate with healthcare professionals to develop and implement care plans for individuals and families.
