About the Role:

We are seeking a highly motivated and skilled Community Health Worker to join our team in the Health Care Services industry. As a Community Health Worker, you will be responsible for providing support and guidance to individuals and families in the community to improve their overall health and well-being. You will work closely with healthcare professionals to identify and address health disparities and promote healthy behaviors. Your role will be critical in ensuring that our community members receive the care and resources they need to lead healthy and fulfilling lives.

Minimum Qualifications:

  • High school diploma or equivalent.
  • Minimum of 1-3 years of experience in public health, mental health, case management, or healthcare support preferred.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.

Preferred Qualifications:

  • Bachelor's degree in public health, social work, or a related field.
  • Certification as a Community Health Worker.
  • Experience working with underserved populations.
  • Knowledge of local community resources and services.
  • Experience with data collection and analysis.

Responsibilities:

  • Conduct outreach and engage with community members to assess their health needs and provide education on healthy behaviors and disease prevention.
  • Collaborate with healthcare professionals to develop and implement care plans for individuals and families.
  • Assist community members in navigating the healthcare system and accessing necessary resources.
  • Maintain accurate and up-to-date records of all interactions with community members.
  • Participate in ongoing training and professional development to stay up-to-date on best practices and emerging trends in community health.

Skills:

As a Community Health Worker, you will utilize your strong communication and interpersonal skills to build relationships with community members and healthcare professionals. You will also use your knowledge of public health and disease prevention to educate and empower individuals and families to make healthy choices. Your ability to work independently and as part of a team will be critical in ensuring that our community members receive the care and resources they need.
