Right at Home Northeast Texas, an award-winning in-home care and assistance company, is seeking to hire registered nurses! Hospice and Home Health nurses excel in this position. This PRN, private skilled role requires someone who can work independently, demonstrate client and caregiver management skills, and have a servant's heart. This includes responding to customer requests in an efficient and professional manner, including clients, families of clients, and referral sources. Experience in infusion, wound care and/or VA referrals are a plus. This position will work 10-20 hours/week.
Job Responsibilities:
- Provide management and supervision of services throughout the approved service area. (mileage reimbursement is available)
- Keep all applicable contacts and/or client files current in written and electronic format.
- Adhere to agency infection control and safety procedures by performing and documenting observations, procedures, and interventions in the home according to the established care plan in a safe and competent manner.
- Effectively communicate and interact with clients/caregivers and families in a supportive manner.
- Interpretation of Agency programs and policies as appropriate to clients and caregivers.
- Participates in the continuing commitment for quality care. Mandatory online continued education courses per quarter.
- Develop and implement a written plan of care for each client with input from Intake. Skilled nursing evaluations/admission assessments, vital sign checks, perform head to toe skilled assessments completed with frequency based on physician orders, availability open to days, evenings and some weekends if delegations or issues arise.
- Evaluate client, family, and agency caregiver(s) response and compliance with care. Update the plan of care to reflect changes in the client/family needs, abilities, and resources according to agency policies.
- Assure the plan of care is reviewed on-site, updated, approved and signed by appropriate agency personnel and the client or designated family member quarterly and as necessary based on changing client needs.
- Coordinate care with DON and/or Care Manager of client duties to include medication refills (with following an in home Medication Reconciliation Record)
- Adheres to Right at Home policies and Procedures.
- Performs work under supervision of Director of Nursing, DON.
- Emergency care as needed.
Required Qualifications:
- Possess a valid driver’s license and current auto insurance
- RN certificate
- Ability to read, write, and understand English as needed for the job
Preferred Qualifications:
- Some experience as a caregiver in healthcare (in homes or facilities)
