They are looking to take on a Clinical Social Worker who will be responsible for conducting psychosocial assessments, developing treatment plans, and offering counseling and crisis intervention to patients facing medical and emotional challenges. This is a hybrid position based in Westchester County, NY.
Clinical Social Worker’s Responsibilities and Duties
- Provide emotional support, crisis intervention, and counseling to patients and families.
- Assess patients' psychosocial needs and develop individualized care plans.
- Collaborate with medical teams, case managers, and community resources to ensure comprehensive patient care.
- Assist with discharge planning and connect patients to appropriate support services.
Clinical Social Worker’s Qualifications and Skills
- Master’s Degree in Social Work (MSW) from an accredited program.
- Certified Social Worker in NY State (LMSW or LCSW).
- 2 years of professional experience in a hospital setting.
- Strong communication, problem-solving, and advocacy skills.
