The ideal candidate will possess a strong background in home health care, with at least 2 years of experience working in a HOME setting within the last 10 years. This includes experience with Certified Home Health Agencies (CHHA), Licensed Home Care Services Agencies (LHCSA), or Managed Long Term Care (MLTC) organizations.
Apart from their clinical expertise, the selected candidate should be able to conduct thorough patient assessments, including Uniform Assessment Instrument for Seniors (UAS) and Outcome Assessment Information Set (OASIS). A valid active RN License in New York State is also mandatory.
