Position Description – What the Role Entails:
The Case Manager will identify individuals in the community who would benefit from education or services offered by the Illinois Welcoming Center (IWC) and Southern Boarder Arrivals (SBA) Programs in Chicago, IL. The case manager will function as a conduit to engage, educate, and promote the IWC program to community members or stakeholders.

Areas of Accountability - What you will be doing:
- Act as a point of contact for the local community, including but not limited to hospitals, health care providers, emergency responders and social service providers.
- Provide comprehensive and culturally sensitive case management services to all participants, including intake assessment, benefit assessment, financial goal setting, educational goal setting, money management, and financial literacy training.
- Regularly engage individuals who need the IWC/SBA program or services from the IWC collaborative.
- Follows procedures during a crisis intervention of medical and/or psychiatric emergencies on and off site.
- Monitor behavioral functions and report as needed to ensure a safe, welcoming environment for participants and staff on site.
- Assist in the referral and linkage to behavioral health or social services in the community.
- Document accurately, neatly, and consistently every and any contact with or on behalf of a participant in the participant’s designated chart, as required by funding source.
- Work in conjunction with agency staff to ensure that appropriate referrals are made, and follow-up services are provided as needed.
- Connect families with needed and available community resources. Follow up with referrals agencies to ensure the use/success of the referral.
- Weekly case plan development, progress monitoring, advocacy, and referrals.
- Provide linkages for participants to community-based services as needed; stay abreast of staff changes and referral procedures at partner agencies.
- Participate in staff meetings, staff training, and other duties as assigned by the program supervisor.
- Transport participants if needed to locations within the greater Illinois area.

Requirements:The Knowledge and Expertise that Matters Most - What you bring to the Role:
- Excellent communication and interpersonal skills (verbal, written).
- Case Management and Outreach experience.
- Be flexible for potential second and/or third shifts as needed.
- Comprehensive understanding of substance use, mental health and HIV/AIDS
- Highly organized and detail oriented.
- Learn technology quickly.
- Ability to engage in many projects at once.
- Ability to juggle multiple tasks, set priorities and follow-through on daily tasks in a timely fashion.
- Ability to handle confidential information professionally and appropriately.

Position Qualifications – What is required to Perform the Role:
- High School Diploma or GED; enrolled in an Associates/Bachelor’s program.
- At least 1 year of previous case management experience is preferred.
- Knowledge and understanding of the local community and its stakeholders.
- Have a vehicle, valid driver’s license, vehicle insurance, and a safe driving record.
