Job Overview
The Hospice Admissions Director is responsible for overseeing and coordinating the patient admissions process to ensure a smooth, timely, and compassionate transition into hospice care. This individual serves as a key liaison between referral sources, clinical teams, and the marketing department, ensuring exceptional service and communication throughout the patient onboarding experience. The role requires strong leadership, relationship management, and operational coordination skills.
Key Responsibilities:
Admissions Coordination:
- Oversee the entire hospice admissions process, ensuring patients and families receive timely, respectful, and comprehensive guidance.
- Collaborate with clinical staff to assess patient eligibility and expedite admission decisions.
- Ensure compliance with hospice regulations and organizational policies during admissions.
- Assist in the scheduling of appropriately skilled person to fill the case, inputs scheduling information on the computer.
- Check common working files for Medicare Coverage and verification of benefit period.
- Answer phones, receive all referral/intake information.
Referral Source Engagement:
- Build and maintain strong relationships with hospitals, skilled nursing facilities, physicians, case managers, and other community referral sources.
- Conduct regular visits and presentations to educate on hospice services and streamline the referral process.
- Act as the primary point of contact for referral partners during the admission process.
Team Leadership & Direction:
- Coordinate with marketing staff to align outreach strategies with patient care needs and referral trends.
- Work closely with clinical leadership to ensure resource availability and readiness for new patients.
- Participate in interdisciplinary team meetings to provide updates and insights on admissions and referral activity.
Strategic Input:
- Analyze referral data and admission trends to help shape marketing and growth strategies.
- Provide feedback to leadership on barriers to admission and opportunities for service improvement.
- Assist in the development and implementation of admission-related policies and training.
Qualifications:
- High school graduate or equivalent.
- Minimum of 3 years of experience in hospice (required).
- Minimum of 1 year experience in care coordination or admissions.
- Strong understanding of hospice philosophy, services, and regulatory standards.
- Excellent interpersonal and communication skills.
- Proven ability to build relationships and lead cross-functional teams.
- Must be computer literate and able maintain simple records.
- Must be able to respond quickly and compassionately in sensitive patient/family situations.
Work Environment:
- Position may be stressful at times.
- It is primarily a desk job which essentially involves sitting, standing, stooping, and walking, as well as telephone communication. Some travel to referral sources, and in-person visits to patient care locations as necessary.
- It requires minimal lifting of office records and printouts.
