Job Description
As a Care Management Professional, you will be responsible for delivering compliant, high-quality, and cost-effective healthcare while achieving positive clinical outcomes and patient/family satisfaction on your assigned unit.
Key Responsibilities:
- Collaborate with the Director of Nursing to maintain adequate nursing coverage for safe nursing care.
- Provide professional guidance and supervision to non-professional unit staff.
- Identify educational needs of staff and develop training with the Nurse Educator.
- Oversee medication management to ensure adequate supplies and adherence to company policy.
- Provide direct patient care to maintain skills.
Qualifications:
- Graduate of an accredited school of licensed practical nursing.
- Current licensure by the State Board of Examiners of Nurses in the center's state.
