Job Summary
As a Pharmacist specializing in Home Infusion Therapy, you will oversee and perform clinical and distributive pharmacy activities to ensure safe, effective, and high-quality care. You will collaborate with physicians, nursing staff, and other healthcare professionals to plan and implement home infusion therapies, provide patient and caregiver education, and monitor therapy outcomes.
Key Responsibilities
- Adhere to organizational policies and establish safe infusion therapy practices.
- Develop and oversee protocols for infusion therapy handling in pharmacy, transport, and patient homes.
- Facilitate new patient admissions and referrals in collaboration with nursing and customer service teams.
- Conduct and document thorough reviews of new patient orders.
- Assess and address patient-specific drug therapy needs.
- Monitor drug therapy outcomes, including evaluating lab and clinical data.
- Communicate findings with healthcare providers to optimize patient care.
- Program infusion control devices as needed.
- Educate patients, families, and healthcare professionals about drug therapies.
- Prepare reports and participate in staff and agency development activities.
- Participate in an on-call rotation to ensure continuous patient care.
Minimum Requirements
- Bachelor’s Degree or higher in Pharmacy
- Valid Pharmacist license in Texas
- 1 years of experience in plasma-derived therapy
- At least 1 year of experience in any of the following: ICU, Critical Care, Clinical Pharmacy, or Pharmacy Informatics
- Strong knowledge of home infusion therapy practices
- Excellent communication and collaboration skills
- Dedication to ensuring safe and effective patient care
