Job Summary

We are seeking an experienced and compassionate Adult or Family Nurse Practitioner to work in Waterloo, NY. The ideal candidate will provide high-quality, patient-centered care and work closely with a multidisciplinary team to enhance the well-being of long-term care residents.

Key Responsibilities

• Provide primary and specialized healthcare services to long-term care residents.
• Assess, diagnose, and treat patients with acute and chronic conditions.
• Develop and implement individualized care plans in collaboration with physicians and nursing staff.
• Monitor patient progress and adjust treatment plans as necessary.
• Educate patients and their families on disease prevention, treatment options, and health maintenance.
• Maintain accurate and up-to-date patient records in accordance with regulatory standards.

Minimum Requirements

• Board Certification
• NYS License
• Strong clinical and diagnostic skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and collaboratively within a multidisciplinary team.
