Base Salary : $115,000 - $135,000 (USD)

Job Summary We are seeking an experienced and compassionate Adult or Family Nurse Practitioner to work in Waterloo, NY. The ideal candidate will provide high-quality, patient-centered care and work closely with a multidisciplinary team to enhance the well-being of long-term care residents. Key Responsibilities

Provide primary and specialized healthcare services to long-term care residents.

Assess, diagnose, and treat patients with acute and chronic conditions.

Develop and implement individualized care plans in collaboration with physicians and nursing staff.

Monitor patient progress and adjust treatment plans as necessary.

Educate patients and their families on disease prevention, treatment options, and health maintenance.

Maintain accurate and up-to-date patient records in accordance with regulatory standards.

Minimum Requirements

Board Certification

NYS License

Strong clinical and diagnostic skills.

Excellent communication and interpersonal skills.

Ability to work independently and collaboratively within a multidisciplinary team.

Ideal Qualifications

Experience in a long-term care or similar setting preferred.
