What are the responsibilities and job description for the Advanced Practice Provider Emergency Department RN FT position at Rise Community Hospital Llc?
Summary: Advanced Practice Provider assists with all aspects of patient care, including diagnosis,
treatments and consultations. Emergency Department RNs are responsible for maintaining care standards in the emergency room. Able to assess patients quickly, prioritize, and assist in minor procedures. Manage patient check-in process, performance of triage work for doctors.
Essential Functions:
A: Job Specific
Critical thinking to intervene with appropriate intervention for urgent/emergent care. Care of the acute and chronically ill patients.
Basic IV and central line skills.
Identifying and managing life-sustaining physiologic functions in unstable patients.
Maintains patient, physician, and employee privacy and confidentiality per policy.
Prepares and administers (orally, subcutaneously, through an IV) and records prescribed medications. Reports adverse reactions to medications or treatments in accordance with the policy regarding the administration of medications by a licensed registered nurse.
Maintains proper supplies and appropriate medical equipment to care for patients.
Displays professionalism while completing multiple urgent tasks in a timely manner.
Maintains an attractive and comfortable environment for patients with special consideration to cleanliness of rooms, equipment and ongoing cleaning of the center
Provide initial evaluation, diagnosis, treatment, and ongoing support for individuals with complex addiction and substance related health conditions.
Complete full and appropriate documentation to support development of integrated care plan and support for encounter data.
Provide education regarding illness and prognosis to assist with clarification of patient and family goals of care and support development of a comprehensive care plan.
Work with an interdisciplinary team of professionals to administer the best care for those patients that need not only medical treatment, but behavioral health services as well.
Demonstrates a strong desire to help patients, families and all referents in the transition to treatment.
Conduct health assessments including recording complete medical and psychosocial history and symptoms
Develop a treatment plan that may include medication and other therapies
Follow up on a patient to determine the effectiveness of recommended treatments
Performs physical examinations and preventive health measures within prescribed guidelines and instructions of Physician.
Orders, interprets, and evaluates diagnostic tests to identify and assess patients’ clinical problems and health care needs.
Records physical findings, and formulates plans and prognosis, based on patient’s condition.
Collaborates with Physician and other health professionals to prepare comprehensive patient care plan as necessary.
Additional Responsibilities:
Maintenance of Rise service line and daily Rise patient assessments.
Daily STO and Inpatient notes. Facilitate STO to inpatient conversions as needed.
Adherence to established detox protocols.
Facility throughput and nursing oversight.
RN distribution of duties and throughput.
Nurse oversight.
Allocation of patients to nursing staff and utilization of medics.
Participate in AM Rise huddles.
Populate and maintain Rise Status board.
Functional charge nurse on shift.
As NP, serve primary nursing function when needed for nursing capacity.
Assume overflow patient nursing responsibilities.
Afternoon rounds with ED physician.
Provider care of ED patients when able.
100% ED physician oversight and cosignatory of charts for full supervision (full revenue capture).
Collaboration with ED physician on all patients.
Engage with Rise consultant as needed.
Execute rapid ED follow-up program with Rise discharge medical follow-up calls when appropriate.
Collaborate with mental health team.
Collaborate with Rise case management and advocate team.
ED Status board updates and maintenance.
