The Community Resource Specialist connects individuals and families, particularly those with Intellectual and Developmental Disabilities (IDD) or other needs, to essential community resources and services. This role involves assessing individual needs, providing referrals, and coordinating support services to enhance independence and overall well-being. The specialist collaborates with service providers, government agencies, and community organizations to ensure individuals receive appropriate assistance.

MAIN DUTIES AND RESPONSIBILITIES
§ Individual Support & Advocacy
§ Assess individual needs and provide appropriate referrals for housing, healthcare, employment, transportation, financial assistance, and other services.
§ Advocate for individuals to access benefits, public assistance programs, and other social services.
§ Assist individuals in navigating complex systems, such as Medicaid, SNAP, SSI/SSDI, and vocational rehabilitation services.
§ Provide crisis intervention and connect individuals with emergency resources as needed.
§ Community Outreach & Partnership Development
§ Develop and maintain relationships with local agencies, nonprofit organizations, and government programs.
§ Create and update a comprehensive database of community resources.
§ Represent the organization at community meetings, networking events, and outreach programs.
§ Conduct presentations and training for staff, community members, and partner organizations.
§ Case Management & Documentation
§ Maintain detailed case notes and documentation in compliance with organizational and funding requirements.
§ Track individual progress, follow up on referrals, and ensure services are effectively meeting individual needs.
§ Prepare reports on resource utilization and program impact.
§ Education & Resource Navigation
§ Provide guidance to individuals and families on eligibility requirements for assistance programs.
§ Assist with applications for financial aid, disability benefits, and housing assistance.
§ Conduct workshops on financial literacy, employment readiness, and access to healthcare.
§ Work with diverse populations, including individuals with disabilities, seniors, low-income families, and those facing housing insecurity.
All other duties assigned.
QUALIFICATIONS
§ Must be 18 years of age.
§ Bachelor’s degree in Human Services or a related field and at least one (1) year of experience in case management, community outreach, or social services, OR Associate’s degree in Human Services or a related field and at least three (3) years of experience in case management, community outreach, or social services.
§ Must successfully complete a pre-employment background check and any subsequent background check after employment.
§ Must have a valid driver’s license and be able to obtain a Missouri Class E chauffeur’s license within 14 days of hire.
§ Successfully complete employment-related training as directed.
SKILLS
§ Excellent communication, interpersonal, and problem-solving skills.
§ Ability to work independently and manage multiple cases effectively.
§ Proficiency in Microsoft Office and case management software.
PHYSICAL REQUIREMENTS
§ Ability to lift up to 25 pounds occasionally for outreach materials and resource distribution.
§ Frequent standing, walking, and occasional bending or reaching when engaging with community members and partners.
§ Ability to travel between community sites, service agencies, and program locations.
§ Work may involve extended periods of sitting while conducting assessments and documentation.
§ Ability to work in diverse settings, including office environments, community centers, and outdoor events.
§ May require occasional evening or weekend work for outreach events and community meetings.
