Under the direct supervision of the Mental Health Assistant Director, the Supportive Housing Program Advisor assumes all operational responsibilities for the Supportive Housing Program, which is comprised of seven scattered sites. Specific attention to Supportive Housing guidelines, contractual agreements, and agency/programmatic procedures are crucial in providing long-term permanent housing options for people with Mental illness.
Duties will include but are not limited to:
- Functions as the “landlord” on behalf of RISE Life Services. It is expected that the Program Advisor display the utmost professionalism, empathy, and can work under minimal supervision.
- Leads the Admission/Eviction process as outlined in RISE protocols- aligned with applicable New York state laws
- Stay abreast of changes within the Supportive Housing program as set forth by the Office of Mental Health (OMH); Initiate team meetings to successfully roll-out the most up-to-date information
- Maintain meticulous records surrounding Sublease, Tenants Rights, Grievance policies, MHRM (Mental Health Recovery Measure); Support Plan/Reviews, Collaterals, and Monthly contact notes for 21 Individuals.
- Meet with each Tenant face-to-face minimally once/month- increasing visits based on need.; Once per quarter, contact must be completed in their home
- Maintain confidentiality; Ensure that consent forms are kept up to date for all Individuals with special attention to their local hospitals and community providers
- Knowledgeable in providing CORE services; Roll-out this initiative within the Supportive Program to increase revenue
- Meet with the Assistant/Operations Director regularly to effectively communicate happenings of the Supportive Program.
- Initiate team meetings to address any violations to the Supportive Housing Sublease; Work in collaboration with Intake Director to issue Notice of Violations.
- Follow up consistently and regularly- without prompting- on all pertinent matters
- Connect tenants to community resources (i.e. Case management; Day Treatment; Employment; Psychiatric/Psychological services, etc.); Be familiar with Section 8 HUD programs and other affordable housing options.
- Verify income information for all tenants, at least once/year; Calculate and submit completed rental stipend worksheets to Operations Director each time tenant income increases or decreases by $50 or more.
- Work collaboratively with all departments within RISE presenting work in a timely, organized, and professional manner.
- Oversee the rental payments/arrears for 21 tenants residing within the program in conjunction with RISE Benefit Specialist
- Attend networking events to stay abreast of changes, and ensure this department is represented
- Submit Check Requests in a timely manner for any monies owed TO a tenant from RISE; Adversely, assist in collection of any payments owed to RISE BY a tenant.
- Assist tenants in actively seeking Section 8/Community permanent housing and applying when opportunities arise
- Innovative Program development based on support needs, interests, and future program direction- under the specific authority of Operations Director
- Provide an outlet for all tenants to share open suggestions; Monitor satisfaction surveys
- Conduct quarterly site-specific “community meetings”; document progress/lack thereof; site-specific concerns; highlight achievements
- Find meaningful ways to reward the Supportive tenants for their successes.
- Maintain present-day annuals & bi-annual documentation necessary in submitting SPOA applications (physical/PPD/psychiatric/Comprehensive Functional Assessment)
- Create avenue for Advocacy and community integration
- Flexible schedule to meet program demands
- Maintain 100% Training Compliance
- Ensures COVID safety practices are implemented; Disseminate COVID-19 products regularly; utilize the electrostatic gun to sanitize the premises at least once/month
- All other duties as outlined/assigned by Assistant/Operations Director
Qualifications
New York State Driver’s License, reliable transportation, B.A. in Human Services preferred, Associates degree or its equivalent in college credits acceptable with at least 2-years of experience working within the Mental Health field. Case Management experience preferred. Knowledge of CORE preferred.
Salary : $24 - $26
