Job Summary:
The Case Manager in the Reentry Field plays a critical role in supporting individuals transitioning back into the community after incarceration. This position involves the development and implementation of personalized ISP - Individual Service Plans that address the unique needs of each client, facilitating their successful reintegration back into society. The Case Manager will provide case planning, goal setting, financial budgeting and support to clients in the Reentry Program which will focus on reducing recidivism by helping clients navigate their reentry and facilitate successful outcomes. The primary duties of the reentry Case Manager are outlined below.
Responsibilities and Duties:
· Carry a case load of up to 24 clients at any given time.
· Meet with clients on a weekly basis for case management services.
· Assist clients in goal setting and planning.
· Create and monitor an Individual Service Plan (ISP) for each client, adjusting as needed.
· Assist clients with creating a budget and track spending.
· Develop client’s savings plan (e.g. monthly savings, retirement account).
· Navigate external systems with clients (e.g., housing, healthcare, transportation, bank account, credit score, probation/parole, etc.)
· Maintain accurate client files and case load statistics related to goals, grants or projects.
· Sustain interdepartmental communication as it relates to client’s ISP and progress.
· Maintain ongoing discussions with clients regarding personal and professional goals.
· Conduct regular performance evaluations to track client’s progress.
· Documentation/Identification assistance: Social Security card, birth certificate, healthcare insurance, California ID/Driver’s License.
· Ensure client seeks and benefits from services offered onsite and from community partners.
· Facilitate contact with probation and parole departments when necessary.
· Maintain accurate and detailed case notes regarding client’s progress or any relevant information to the case planning.
· Attend regular meetings with supervisor to discuss client's progress.
· Coordinate logistics for events offered to client and their families in the community.
· Work closely with machine shop staff to ensure a successful transition from the program into societal employment.
· Answer emergency calls outside of normal work hours and occasional weekends, if needed.
· Occasionally attend RUI events and/or committee meetings either in the evenings or weekends.
· Provide crisis intervention when necessary, helping clients navigate difficult situations and avoid setbacks in their reentry process.
· Review and update weekly errors on client’s time sheets.
· Update weekly machine shop attendance folder.
· Other duties as assigned.
Requirements:
· Minimum of 2 years experience in case management, social services, restorative justice or related fields. Experience working with individuals transitioning from incarceration or experiencing reintegration is highly preferred.
· Strong understanding of the criminal justice system, reentry challenges, reentry programs, available community resources and support networks for the formerly incarcerated.
· Excellent communication, problem solving and organizational skills.
· Ability to work independently while collaborating with multidisciplinary team.
· Strong interpersonal and communication skills with the ability to build rapport and trust with clients from diverse backgrounds.
· Commitment to fostering a supportive and non-judgmental atmosphere for clients.
· A valid driver’s license/vehicle insurance and access to reliable transportation may be required.
