ESSENTIAL DUTIES AND RESPONSIBILITIES
The Nursing Supervisor will monitor all phases of Nurses Operations and ensure that all appropriate procedures are in compliance with the program compliance requirements. Review of quality of work performance and addresses areas in need of improvement. Act as a liaison between the Agency, hospitals and/or other side offices. Additional duties and responsibilities include:
- Perform random clinical review of client files.
- Perform initial home care assessments and re-evaluations
- Observe, assess, monitor and document symptoms and client's progress
- Educate clients and caregivers on techniques in home health care
- Supervise RN’s, LPNs and HHA’s in the field
- Notify physician and other personnel of change in client's condition
- Case conference clinicians to ensure coordination of care
- Update clinical records according to NYSDOH and agency policy and procedures
- Provide onsite supervision, training and support of RN’s, LPN’s and HHA’s
- Attends in-service programs, continuing education programs and seminars annually
- As necessary, conduct initial home visits to assess client needs and develop the Plan of Care
- Communicate with a client’s physician and other team members to ensure delivery of appropriate client care
- Supervise client care provided by our Home Health Aides
- Participate in orientation, teaching, and evaluations of care providers
- Responsible for the review and maintenance of client records, using telehealth modalities for early intervention of client issues
- As necessary, provide client/families with information regarding agency services, fees, Bill of Rights, Advance Directives and agency policies.
- Communicate with referrers/discharge planners to review eligibility for agency services and to develop and coordinate the plan of care.
- Provide assessment of newly referred clients, assess health needs and eligibility for home care. Obtain client’s medical history, particularly as it relates to the client’s present condition.
- Identify the appropriate level of care required by the client. Ensure the environment is conducive to the provision of home care.
- Develop an individualized Plan of Care for the client based on needs and findings of the nursing assessment.
- Coordinate all care and services provided to client.
- Monitor the client’s medication regimen for drug-drug/drug-food interactions, side effects and adverse reactions.
- Perform client re-assessments in the home. Reassess client needs for services on a continual basis and discuss any changed in the plan of treatment with client, physician/family.
- Review reports from all involved disciplined, re-evaluating needs and Supervise all involved disciplines in the plan of care.
- Conduct on-site competency review as part of supervision activities.
- Prepare written documentation and summaries of the client’s status and progress as needed for continuation of home health services.
- Initiate referrals for other services or community resources as needed.
- Maintain updated professional knowledge and participate in client education programs to ensure optimum quality of client care.
- Participate in the agency’s in-service program. Provide in-service education to professional/paraprofessional staff.
- Participate in the agency’s QI Program, annual policy/procedure review, as requested. Perform Quality Improvement duties, completes audits, evaluated results with DPS recommendations for quality improvement.
- Perform annual field visits/evaluations on para-professional and nursing personnel.
- Consult with DPS to maintain the highest quality care. Report significant occurrences to the DPS relating to client care.
- Review and respond to complaints, grievances and incidents.
- Interpret and reinforce agency policy and procedures to professional and paraprofessional staff.
- Provide 24-hour on-call coverage on a rotation basis
- Consults physicians in matters relating to patient care services.
COMPETENCIES
- Must have an active New York State RN License
- 1 years of experience working in a Hospital & Home Care setting
- Excellent verbal and written communication skills
- Excellent interpersonal skills

- Excellent organizational & time-management skills
- Knowledge and understanding of Home Care policies and procedures
- Preferred but not required experience working in a LHCSA
- Excellent organizational & time-management skills
- Basic computer skills; knowledge of data base entry and email (i.e. Outlook)
- Excellent documentation skills, ability to meet deadlines
- Ability to function independently with minimal supervision
EDUCATION and EXPERIENCE
NYS licensed and currently registered as a Registered Professional Nurse
