Care Coordinator (Per-Diem)

RiseWell Community Services is looking for a Per-Diem Care Coordinator to become part of our dedicated Care Coordination Department in the Long Island region. This role offers:

In this role, you will Provide care coordination and health promotion services to individuals throughout Long Island. Provide Care Coordination to those with severe mental illness and/or 2 or more chronic health conditions to avoid costly inpatient hospitalizations, emergency room visits and unnecessary care. Assist individuals in obtaining Food Stamp and Social Security benefits. Oversees and coordinates access to all services (behavioral, medical health and social service supports) to assist individuals to stay healthy. Completes all required documentation according to standards and no later than 24 hours of service delivery.

QUALIFICATIONS:
Minimum of a Bachelor’s Degree and 2 years of relevant experience serving people with mental illness, developmental disabilities or alcoholism or substance abuse. Clean, valid NYS Driver’s License preferred. Fingerprinting, criminal record check, approval from NYS Office of Mental Health. Spanish speaking preferred.
