Health Counselor - LPN - OASAS Methadone Clinic

RiseWell Community Services is looking for a Licensed Practical Nurse for our Clinical Department, working in RiseWell’s new branch of Substance Use Services in the Methadone clinic. Our Methadone clinic is a crucial addition to the county in treating and helping individuals struggling with opioid addiction manage withdrawal symptoms and allow them to focus on recovery.

In this position, you will be providing management and counseling to a caseload of program members, in conjunction with the psychiatrist and registered nurse. As a part of an interdisciplinary team dedicated to treating personals with substance use disorders, you’ll be working to improve the health and quality of life of a caseload of program members.

Our Licensed Practical Nurse should be a dynamic and proactive individual who excels independently but also as a collaborative team member. Ideally, you are a self-starter with a commitment to making a meaningful impact.

QUALIFICATIONS:

· Currently licensed as a Licensed Practical Nurse by the New York State Education Department

· Minimum 1 year experience in a treatment or rehabilitation setting preferred

· Valid and clean NY State driver license

· Fingerprinting, criminal record check, approval from OASAS

· Strong interpersonal skills

· Effective written and verbal communication skills

CORE & ESSENTIAL RESPONSIBILITY:

· Provide health management and counseling to help individuals in mental health and substance use recovery

· Work effectively in conjunction with the psychiatrist and registered nurse, as well as others on the treatment team for persons with substance abuse disorders

· Ensure medical standards as per program regulations are being met

· Responsible for medication management, education, and administration

· Attend all assigned meetings

· Conduct all assigned visits

· Oversee the accurate completion of all documentation

· Assist in the maintenance of the center and ensure the environment promotes recovery

· Provide education to clients and staff regarding standards of medical care

· Responsible to comply with agency Compliance plan and False Claims Policy by preventing, detecting and reporting any potential healthcare abuse and fraud. Report any potential non-compliance and comply with all other federal, state and local regulations applicable to this job description.

· Responsible for proficiency in all agency based IT applications.

· Responsible to perform all other job duties as assigned.
