RiseWell Community Services is looking for (Two) full-time Substance Abuse Specialists to join our ACT & the Forensic Assertive Community Treatment (FACT) team.
This individual will supervise and provide direct services, with a focus on substance abuse recovery and harm reduction, to individuals with severe and persistent mental illness. The Substance Abuse Specialist serves as a member of a mobile, multi-disciplinary treatment team, delivering individualized care to those in need.

Qualifications
- Master’s degree or higher in Social Work, Psychology, or Rehabilitation Counseling (if Rehabilitation Counselor, C.R.C. preferred)
- Minimum 1 year of experience in a treatment or rehabilitation setting
- A valid and clean NY State driver license
- Fingerprinting, criminal record check
- Approval from NYS Office of Mental Health
- Strong documentation and organizational skills
- Effective time management skills
- Knowledge of treatment and rehabilitation methods for persons with severe and persistent mental illness
- Skilled with individual and group counseling techniques
- Maintain case records and assume responsibility for caseload assigned by the Team Leader
- Manage referrals and intake assessments in accordance to agency timeline and at the discretion of the Team Leader
- Identify individualized treatment plan based on substance use diagnosis, immediate needs, and progress of change
- Conduct daily visits in the community or based on requirements from the Team Leader
- Collaborate, complete, and review all updates at a minimum of every 6 months including Treatment Plan Review, Comprehensive Assessment Update, Crisis/Wellness Plan, Substance Abuse Assessment, Psychiatric Evaluation, Physical Health Assessment, and more
- Complete weekly progress updates
- Complete visits based on requirements from Team Leader
- Prepare and submit reports by deadlines
- Perform other duties consistent with participant’s goals and as directed by the Team Leader
