Ritter Center is recruiting a Medical Case Manager for our Whole Person Care Department. This full-time, non-exempt position reports to the WPC Program Manager and is based in San Rafael, CA; the role operates out of our Administrative Offices.

Position Description: The Whole Person Care Medical Case Manager functions as a medical healthcare manager responsible for the coordination of medical and dental services for high-risk and complex Whole Person Care (WPC) program individuals.

The individual in this role acts as a comprehensive care coordinator for patients to assist them in accessing medical and specialty care services, ensuring the patient is regularly assessed, transportation needs are addressed, and communication among clinicians and agencies providing care is properly facilitated.

Job Duties and Responsibilities:

  • Handles sensitive information confidentially, abiding by HIPAA requirements
  • Facilitates communication between clients, Ritter clinic staff, specialty pharmacy, and specialty care practices as needed
  • Assists with new patient intake as well as other aspects of patient care, including prior authorizations
  • Conducts medical and psychosocial client assessments as needed
  • Practices safety, environmental, and infection control methods
  • Works and completes tasks in EClinical Works electronic health record and Wizard (County record) in a timely manner
  • Works with care team to develop a comprehensive health care plan that includes setting of patient goals for health and psychosocial needs
  • Works with other members of individuals care team (IHSS, MCH, recovery coaches, BHRS, etc) and/or makes referrals to services as appropriate
  • Meets with all enrolled individuals twice a week, at least one of which is a face-to-face meeting, to assess medical needs and progress toward goals
  • Conducts screenings and assessments for Housing-based case managers as prescribed in the WPC contract
  • Assists the patient with making medical and specialty care appointments, transportation needs and when appropriate, accompanying patient to medical appointments to serve as an advocate
  • Documents contact in Wizard and ECW (electronic health records)
  • Ensures appointments are scheduled and facilitates transportation if needed for each scheduled patient
  • Prepares quarterly and annual reports on service utilization, treatment outcomes, and narrative of the overall grant program
  • Participates in Quality Improvement activities, including data collection and reporting
  • Completes and submits all required documentation required by external agencies
  • Demonstrates appropriate judgment skills to be able to make independent decisions in routine patient care matters
  • Remains flexible and responsive when changes occur in patient activity and workload
  • Other duties as assigned or needed

Qualifications:

  • Per OSHA requirements employment at Ritter Center requires compliance with being able to produce a negative TB test prior to the start of employment, as well as being fully vaccinated against COVID-19 (unless applying for an exemption) per the August 5th, 2021, State Public Order for Healthcare Facilities, providers and employees.
  • Strong organizational skills and ability to multitask, especially under pressure
  • Basic computer knowledge required, accuracy in data entry
  • Graduate of an accredited College, University with an Associate’s degree or higher
  • Experience or strong interest in working in a health care setting
  • Demonstrated effective written and oral communication skills
  • Current BLS certification or willing to become certified
  • Experience working with vulnerable clients, as well as low income or homeless populations.
  • Experience working in social service setting; ability to be empathic and compassionate with clients combined within a framework of appropriate boundaries
  • Understanding of the needs and the range of conditions and circumstances experienced by low income, working people and people experiencing homelessness or co-occurring conditions
  • Works well in a team environment
  • Basic computer knowledge required, as well as accuracy in data entry and competency in navigating multiple computer programs
  • Must have a current, valid Class C driver’s license and a personal vehicle for work transport
