ABOUT THE ROLE:
The Community Engagement Coordinator will play a critical role in supporting and coaching Community Health Navigators (CHNs) to enhance community health initiatives, particularly among underserved populations. This individual will work closely with CHNs, healthcare providers, and community organizations to implement outreach programs, provide training, and ensure that culturally relevant health education is effectively delivered. The position requires a strong commitment to health equity, relationship-building, and empowering CHNs to drive meaningful change in their communities.
KEY ACTIVITIES:
Community Health Navigator (CHN) Support & Coaching
- Provide ongoing training, mentorship, and professional development opportunities for CHNs.
- Assist CHNs in implementing effective community outreach and engagement strategies.
- Ensure CHNs have the necessary resources and materials to educate and support community members.
- Monitor CHN performance and provide guidance to enhance their impact in the field.
Community Engagement & Outreach
- Develop and maintain relationships with community organizations, healthcare providers, and stakeholders.
- Support CHNs in organizing and facilitating community health screenings, workshops, and events.
- Assist in developing culturally appropriate health education materials and resources.
- Identify and address barriers to healthcare access within the community.
Program Coordination & Implementation
- Work collaboratively with leadership to align CHN activities with organizational goals.
- Track and evaluate CHN engagement efforts and program effectiveness.
- Collect and report data on outreach activities, health outcomes, and community needs.
- Develop strategies to expand CHN-led initiatives and increase community participation.
Advocacy & Capacity Building
- Advocate for policies and programs that support CHN integration into community health initiatives.
- Identify and implement best practices for strengthening CHN roles in healthcare delivery.
- Facilitate networking opportunities for CHNs to collaborate and share experiences.
QUALIFICATIONS:
- Bachelor’s degree in Public Health, Social Work, Health Education, or a related field.
- 2 years of experience in community health, outreach, or working with CHNs.
- Bilingual (Spanish or another language spoken in the community) is highly preferred.
- Strong understanding of health disparities, social determinants of health, and community-based interventions.
- Experience in coaching, mentoring, or training staff or volunteers.
- Excellent communication and interpersonal skills, with the ability to engage diverse populations.
- Ability to work collaboratively with stakeholders, including healthcare providers and community organizations.
- Proficiency in data collection, program evaluation, and reporting.
- Must be able to travel within the community and occasionally work flexible hours, including evenings or weekends.
REQUIREMENTS:
Communication Requirements:
- Bilingual/Multilingual in language of the targeted immigrant community.
Reasoning Ability Requirements:
- Professional-level, reasoning abilities are necessary to process complex information.
- Candidates must be able to delegate duties.
- Being able to stay organized and effectively prioritize deadlines.
Other Requirements:
- Able to use technology essential to job responsibilities (computer software, data information systems, fax and internet)
- Valid driver’s license, auto insurance and a good driving record
- Reliable mode of transportation required
- Must pass a background check
- Must be able to work some nights and weekends.
- Occasional lifting up to 15 lbs. may be necessary from time to time.
- Must be able to stand and/or sit for hours at a time
Job Types: Part-time, Contract
Pay: $15.00 - $25.00 per hour
Expected hours: 20 per week
