Ensuring that individuals are fully integrated into the community. Observe staff interactions with individuals and peers. Provides training, coaching, and mentoring to lead and direct support staff and provide transportation.
Responsibilities
Attend ISP meetings (annually, at day center)
Monthly home meetings
Training of direct support staff on day one of in-home orientation
Assist Lead with Financial management of individual funds.
Observes and monitors client behavior to provide feedback in ISP meetings.
Counsels and advises clients on behavioral problems, daily decision-making, and the resolution of minor problems as needed.
Provides transportation for individuals to medical appointments, as needed.
Act as advocate for residents with Support Coordinators, other providers and the community as needed.
Possible Flex schedule to cover staff call outs, holiday coverage as needed.
Approves timesheets weekly, ensure time is flexible to not work over 40 hours.
Develop monthly staff schedule for home/ everyone.
Meet with lead staff weekly for coaching and mentoring.
Present during Audits from external organizations (HFR, Delmarva, GMCF, etc.)
Ensure CAPs are followed up in each home.
Communicate with family members as needed.
On call one week per month
Technical Competencies
Ability to observe and monitor participants’ behavior.
Ability to provide coaching and mentoring.
Knowledge of agency confidentiality policies.
Knowledge of individualized service plans.
Knowledge of the documentation process.
Ability to communicate both orally and in writing.
Knowledge of agency safety policies and procedures.
Conflict resolution skills.
Ability to identify disruptive behavior.
Ability to recognize contributions and celebrate accomplishments.
Other duties as assigned.
Requirements:
Bachelor’s Degree in related field with 1 year experience
Valid Driver’s License with reliable transportation
Acceptable Driving record
Acceptable background check
Must pass the drug screen.
Current physical/ TB test
Acceptable references.
Ability to pass basic CPR and first aid.
