In this role you will:
- Provide support to individual clients and their families.
- Facilitate group, including process skill developing and providing education on relevant topics including symptom management, daily living skills and community living skill development.
- Help Individual to develop self- advocacy skills to achieve decreased dependency on mental health system.
- Use a strengths-based approach to work collaboratively with clients and River Edge treatment team members to help support clients in their mental health treatment.
- Provide referral resources, mentors, family members, and others to help support clients in their treatment.
- Model and support health and wellness activities and healthy coping skills for clients.
- Maintain quality documentation including case notes, discharge planning, and client satisfaction Survey; enter necessary information into River Edge's electronic medical records (EMR) system.
- Other assigned duties as requested.
- Acts as client advocate in order to coordinate required services or to resolve emergency problems in crisis situations.
- Collects information about clients through interviews and observations.
- Utilizes skills trainings with clients in both individual and group sessions in order to help them overcome deficiencies, recover from illness, and better adjust to life.
- Develop and implement treatment plans based on clinical experience and knowledge.
- Insures confidentiality of records relating to client’s treatment.
- Evaluates client’s physical or mental condition based on a review of information.
- Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plans.
- Insures safety and security of clients and co-workers. Must have good knowledge of agency safety policies and procedures.
- Completes doc via EMR in a timely manner.
- Other duties as assigned.
- Bachelor’s degree in related field and two years, Preferred of related experience or a Master’s degree in a related field. Or one year at a lower level or equivalent position.
- Completes/maintains continuing education requirements. Examples include: Mindset CPR/FA/AED, Essential Learning, and others as required.
- Good interpersonal skills. Has the ability to observe, record, and report deviations in patient’s conditions.
- Good communications skills, both from an interpersonal standpoint and the ability to write/produce written reports/documentations as needed to insure good patient care.
