ACT is an Evidence Based Practice that is person-centered, recovery-oriented, and a highly intensive community-based service for individuals who have serious and persistent mental illness. The individual’s mental illness has significantly impaired his or her functioning in the community. ACT provides a variety of interventions twenty-four (24) hours, seven days a week. The ACT Team works as an organizational unit providing community-based interventions that are rehabilitative, intensive, integrated, and stage specific.

- Provide individuals and families with the psychosocial support needed to cope with chronic and acute mental illness in client homes, in public places, outpatient settings, and/or inpatient clinics.
- Provide clients with education and counseling to clients and their family members.
- Advise caregivers and make necessary referrals for other social services.
- Continually build trusting relationships with clients to help drive favorable outcomes.
- Supervise clinicians and support staff within the scope of the ACT program.
- Ensures all standards and regulations of the ACT program are met.
- Complete behavioral health assessments, treatment plans, and authorizations for services.
- All ACT team members participate in a rotating on call schedule.
- Maintain all certifications required by River Edge
- Other duties as assigned.
- Master's degree in social service field (e.g., Social Work, Sociology, Psychology,) or related field, such as counseling required.
- Licensure required in one of the following: LAPC, LMSW, LAMFT, LMFT, LPC, LCSW) required.
- Two years of supervisory/management experience of clinical and support staff.
- Two years of experience of documented experience working with adults with Severe and Persistent Illness.
- Valid Georgia Driver’s license
- All applicants must pass a satisfactory background clearance and pre-employment drug test.
- Provide 2 Professional references (Former employers only, no family members).
