Description

We are seeking a passionate and community-driven Behavioral Health Outreach Specialist to join our team. This role will focus on supporting diverse communities by navigating families through behavioral health resources, conducting outreach to build partnerships, and empowering families with the tools and services they need.

Program Summary:

  • The Flexible Integrated Treatment (FIT) Program serves infants, children and youth (ages 0-21) with a team approach to treatment planning allowing clinicians the flexibility to assess the level and type of services needed. As treatment needs vary over time, services are tailored to fit without interruption. Equally flexible is the location of service: home, school, River Oak office sites or community setting to best accommodate the family. A wide array of treatment interventions, to include multiple evidence-based programs, are used to include individual, family and group therapy.
     

Job Responsibilities:

  • Navigation (40%)
  • Learn and understand the values, beliefs, and practices of diverse cultural groups.
  • Advocate for the community while acting as a cultural guide for families.
  • Ensure services are accessible, equitable, and meet families where they are.
  • Conduct screens and assessments with clients and caregivers to determine needs, including Medi-Cal eligibility and housing resources in collaboration with the Department of Health Administration (DHA).
  • Empower families in problem-solving and accessing community resources.
  • Serve as a liaison for families when engaging with community services and agencies, offering language support services when needed.
  • Maintain accurate progress notes within the electronic health record system within established timelines.
  • Outreach (50%)
  • Conduct outreach efforts in identified communities to enroll children, youth, and adults in need of behavioral health support.
  • Build relationships with local county agencies, schools, clinics, and neighborhood partners to increase awareness and resource availability.
  • Follow up with community partners to maintain a strong referral pipeline to behavioral health and other services.
  • Participate in community resource fairs and local events to connect individuals with services.
  • Additional Duties (10%)
  • Maintain a safe, clean, and enriching environment for families and staff.
  • Advocate for clients while collaborating with other programs and agencies.
  • Assist with outreach, community engagements, referrals, and intake.
     

Minimum Qualifications:

  • Must reflect the diversity of the neighborhoods served, as identified in the Sacramento County contract.
  • Knowledge or willingness to learn about systems of oppression and their impacts on families.
  • Open to learning, self-reflection, and engaging in cultural humility.
  • Basic computer skills with a willingness to learn virtual service platforms.
  • Strong interpersonal skills with the ability to work with diverse workgroups and serve families of various backgrounds.
  • Knowledgeable about local community resources and proactive in learning more.
  • Ability to establish personal and programmatic boundaries while providing supportive services.
  • Effective verbal and written communication skills.
  • Must be able to manage stressful situations and work independently on multiple projects.
  • Physical ability to frequently bend, twist, squat, kneel, reach, push, and lift/carry up to 15 lbs.
