Program Summary:
River Oak's High-Fidelity Wraparound (WRAP) program offers a unique, non-traditional team approach to community-based mental health treatment while partnering with youth (ages 0-21) and their families with complex behavioral needs. Wraparound Care Coordinator will be working as part of a treatment team, providing empathetic, non-judgmental and creative clinical coordination of care to individual youth and their families that are consistent with the mission and values of the Wraparound Process. In order to individualize services for each family's specific strengths and needs, Wraparound can offer a wide array of treatment interventions, which include multiple evidence-based programs (DBT, TFCBT, etc.) for individuals, family and groups. Wraparound is flexible and accessible to our community with the location of service offered in home, school, River Oak clinical sites or community setting to best accommodate the family.
WRAP is an intensive child-focused and family-centered approach driven by individualized needs to wrap services around a child, aged 3-18 years, at risk of losing their current placement or in order to return a child or youth from residential care to family setting. WRAP uses a collaborative team approach to bring a wide array of natural community supports and professional services to allow a family to build on their strengths. The team develops, facilitates and continually evaluates a comprehensive plan of services and resources. Children or youth place out of county or out of state are successfully reintegrated into a welcoming Sacramento home.
Job Responsibilities:
- Provide strength-based and individualized coordination of care with youth, families, internal team members and community partners to affect positive changes
- Lead Child and Family Team Meetings
- Participate in on-going consultation, coordination of care, required documentation, and systematic support with treatment team in the implementation of Plan of Care for individual youth and families
- Perform home, school, office and community visits to observe and assess the youth in the appropriate setting(s); plan and participate in activities; listen to the youth and/or family in a supportive and outcome-based manner; empower the youth and family; perform crisis management and on-call duties
- Maintain HIPAA compliant electronic systems and records
- Meet established billing goals; track productivity levels on a consistent basis
- Maintain awareness of and participation in ongoing professional development
- Practice cultural consideration, concern and courtesy to youth, families, community partners, and other staff members when giving appropriate information, advice, and assistance
- Maintain professional relationships with all team members
Minimum Qualifications:
Must have at minimum:
- Bachelor's degree from an accredited university or college with major course work in social work, psychology, counseling, or closely related field.
- PLUS at least four (4) years full-time experience in providing mental health services/social services case management or mental health counseling; services must be provided in mental health, community-based, or probation setting;
- Please note: **Experience in an academic counseling setting does not qualify**

OR
- Possess a Masters Degree from an accredited university or college with major course work in social work, psychology, and counseling or a closely related field
- Must have excellent organizational, analytical, communications, and care coordination skills
- Must be able to empower people with a strength-based approach, prioritize responsibilities, and build rapport with a wide variety of individuals
- Ability to meet the professional standards and requirements for assigned duties and responsibilities
- Proficient with Outlook, Microsoft Word, Excel and PowerPoint
- Maintain a variety of online recordkeeping and documentation processes and systems for assigned clinical program; ensure all relevant confidentiality and privacy requirements are met
