MDS Coordinator- River Park Post Acute
Pay: $63K to $90K per year depending on if you are an LPN or RN and your years of experience.
Shift: Mon- Fri 8am to 5pm

What does an MDS coordinator do?
The MDS Coordinator is responsible for ensuring the timely completion of accurate assessments and interdisciplinary care plans that meet Federal and State guidelines. The MDS Coordinator identifies resident acuity and needs, helping to determine specific care needs and communicating needs as well as expectations to families and responsible parties. This role is responsible for completing assessments, Minimum Data Set (MDS) and care plans for all residents assigned.
What are we looking for?
Looking for an RN or LPN to conduct and coordinate the development and completion of the resident Assessment Instrument (RAI), that is, the Minimum Data Set (MDS), Care Are Assessments (CAA's) and care plan in accordance with state and Federal requirements. As the MDS Nurse you participate as a part of the facility interdisciplinary team (IDT) in the systems and process to manage patient receiving skilled services as assigned.

Experience:
- Valid nursing license in the state of employment.
- Prior experience in a similar role preferred but not required.
- Knowledge of HEDIS (Healthcare Effectiveness Data and Information Set) measures is a plus.
- Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare team members.
- Ability to prioritize tasks, multitask, and work in a fast-paced environment.
- Excellent critical thinking and problem-solving abilities.
