Hours: ( 4hours x 5 days or two 8hour days and one 4 hour day)
Job Description:

In this role, you will work closely with residents, families, and interdisciplinary teams to ensure individuals receive the care, services, and support they need.
Key Responsibilities:
- Assist with resident assessments and develop care plans tailored to individual needs.
- Coordinate with healthcare providers, social services, and other resources to facilitate timely and appropriate services.
- Provide emotional support and guidance to residents and their families, helping them navigate care options and address concerns.
- Maintain accurate documentation of interactions, assessments, and care plans to ensure compliance with state and federal regulations.
- Advocate for residents to ensure their needs are met, enhancing their quality of life.
- Support new resident admissions, discharges, and transitions, ensuring a seamless process.
Qualifications:
- A high school diploma or equivalent (Bachelor’s degree in Social Work or related field preferred).
- Experience in social services, case management, or healthcare preferred.
- Strong organizational and communication skills.
- Compassionate and empathetic approach to helping others.
- Ability to work collaboratively with a team and independently when needed.
- Basic computer skills for documentation and reporting purposes.
