Description
The LPN Care Coordinator is a core member of the patient care team. The LPN Care Coordinator functions as a liaison with insurance companies and healthcare providers to arrange testing and/or documentation of care specific to clinical quality measures. The LPN Care Coordinator participates in the multi-disciplinary process of providing care and treatment to patients and provides clinical support and technical assistance. Under supervision of the primary care provider, the LPN care coordinator may also provide coordination of services for assigned panel of patients. The LPN Care Coordinator will participate in delivery of care, coordination of referrals, and provide comprehensive care management. Position requires excellent customer service skills with patients, and their families, other staff, physicians, vendors, payers, and the public.
Requirements
· Completion of State approved course of study for Practical Nurse training. Must maintain knowledge of latest developments in nursing practice techniques.
· Current PA Licensed Practical Nurse licensure.
· Maintains current Cardiopulmonary Resuscitation certification (CPR).
Flu vaccination or proper exemption required
Previous experience in medical office preferred.
