Job Summary
The Registered Nurse plans, organizes, and directs hospice care, emphasizing community health education and experience. As a professional nurse, you will utilize all elements of the nursing process to provide comprehensive and compassionate care to patients and their families within their homes and communities.
Duties & Responsibilities
Patient Care
- Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs.
- Provide professional nursing care using the nursing process.
- Develop care plans that establish goals based on nursing diagnosis and incorporate palliative nursing actions.
- Administer medications and treatments as prescribed by the physician.
- Assist patients with activities of daily living and facilitate their efforts towards self-sufficiency and comfort care.
- Act as a Case Manager and coordinate patient care for assigned caseload.
