Job Summary :
The registered nurse plans, organizes, and directs hospice care, with an emphasis on community health education and experience. As an RN Hospice, you will utilize your nursing skills to provide professional care to patients and their families within their homes and communities. Your responsibilities will include completing comprehensive assessments, developing care plans, administering medications and treatments, coordinating patient care, and providing emotional support to patients and their families.
Duties & Responsibilities :
Patient Care :
- Complete initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
- Provide professional nursing care utilizing the nursing process
- Develop care plans based on nursing diagnosis and incorporate palliative nursing actions
- Administer medications and treatments as prescribed by the physician
- Assist patients with activities of daily living and facilitate self-sufficiency and comfort care
- Act as a case manager and coordinate patient care
