Work Location: Fort Smith but willing to travel to any RVPCS location as needed.
Reports To: Medical Director
**Safety Sensitive**
Essential Functions:
1. Incorporates evidence based guidelines and performance measures in delivering clinical services and provides high quality supervision of patient care
2. Appropriate and timely diagnoses based on findings of the current history and physical exam
a. Medication review and update
b. Appropriate ordering of diagnostic tests
c. Appropriate management of patient referrals
d. Interprets and integrates data to determine diagnosis and therapeutic plan.
3. Administers medications and injections in compliance with law and clinic guidelines. Performs procedures within scope of practice
4. Instructs patient/family regarding medications and treatments. Educates patients regarding health and illness prevention. Recommends community resources to meet patient and family needs.
5. Collaborates with physicians in managing acute and long-term medical needs of patients. Provides monitoring and continuity of care between visits. Maintains and reviews patient records, charts and other pertinent information. Posts tests examination results. Triages patient telephone calls and provides consultation.
6. Properly documents all patient information and notes in the medical record. Medical records are completed and signed off in a timely fashion.
7. Maintains patient confidentiality in compliance with HIPAA regulations.
8. Supports initiatives such as Patient Centered medical Home (PCMH) and Meaningful Use (MU)
9. Participates and encourages diversity and inclusion within RVPCS.
10. Other duties as assigned.
Education: MSN required. Successful completion of an approved nurse practitioner program.
Requirements:
1. Must be qualified in Basic Life Support techniques
2. Current state RN license.
3. Current APN license.
4. Participate in Quality Assurance Plan
5. Complete CME/literature review as required.
6. Independent Practice license preferred.
Knowledge:
1. Knowledge of professional nursing theory, practices, and regulations to give and evaluate patient care.
2. Knowledge of how to use medical equipment and instruments to administer patient care.
3. Infection Prevention
Skills:
1. Skill in applying and modifying the principles, methods and techniques of professional nursing to provide ongoing patient care
2. Skill in taking medical histories to assess medical condition and interpret findings.
Abilities:
1. Ability to maintain quality control standards listed in the Policy Manual.
2. Ability to react calmly and effectively in emergency situations.
3. Ability to interpret, adapt and apply guidelines and procedures.
4. Ability to communicate clearly and establish/maintain effective working relationships with patients, medical staff and the public.
5. Ability to improve and maintain continuing medical education and literature review.
Experience:
1. Three years of professional nursing experience in a clinic setting plus two years of nurse practitioner experience is preferred.
